Info pamphlet for tangata whaiora: what Unaunahi is, what is shared, and your consent.
Unaunahi is the referral pathway used by CAF to refer to the NGO partner services that sit under Ka Puta Ka Ora / Emerge Aotearoa or Stepping Stone Trust.
Unaunahi means fish scales. Just as each scale overlaps the next to protect the fish, Unaunahi brings NGO service partners together so that no tangata whaiora falls through the cracks.
Forms and guides for CAF case managers, and information to share with tangata and whānau.
